Completing and Submitting the Forms

How do I fill out and submit my New Student Immunization and Tuberculin form?

MIT Health has partnered with Patient First.AI to review your pre-entrance health requirements. You should have received an email from Patient First.AI with instructions for downloading the Patient First.AI app. Once you have downloaded the app, log in by providing your information. An account has already been created for you. 

When you are logged in, you can fill out and upload your vaccination records. If you have difficulties, email contact@patientfirstai.com or use the chat tool within the app.

A paper version of the form is also available for download if your clinician requires one. 

What if I didn’t receive an invitation email from Patient First.AI?

If you have not received an email from Patient First.AI by August 15, check your spam folder first. Then contact Patient First.AI at contact@patientfirstai.com

How do I fill out and submit my Varsity Student Athlete Physical Examination form?

The Varsity Student Athlete Physical Examination form is available on the MIT Health website. Submission instructions are on the form. You do NOT submit the Varsity Student Athlete Physical Examination form through Patient First.AI.

Who must complete the New Student Immunization and Tuberculin form and Varsity Student Athlete Physical Examination form?

All incoming students must complete the New Student Immunization and Tuberculin form, regardless of age or insurance status. This includes undergraduate students, graduate students, and students who are returning to MIT after a leave of absence of a year or more.

The Varsity Student Athlete Physical Examination form only needs to be completed by students participating in their first year of intercollegiate varsity athletics.

I received my undergraduate degree from MIT; now I’m returning as a graduate student. Since I submitted my New Student Immunization and Tuberculin form when I entered MIT as a first-year undergraduate, do I need to submit the form again?

Yes. We need to make sure you are still up to date on necessary vaccines and tests. You can access your vaccine history in the HealthELife portal.

I have waived the MIT Student Health Insurance Plan (MIT SHIP). Do I still have to complete the New Student Immunization and Tuberculin form?

Yes. All entering students must complete New Student Immunization and Tuberculin form, no matter which health insurance they have. 

What if my medical records are not in English?

There is no need to translate your forms. Patient First.AI, our vendor, will review and translate your documents.

My doctor gave me a paper with my vaccine records. How should I submit this form?

Download the Patient First.AI app from Google Play or the App Store, log in, and follow the instructions within the app. You can email questions to: contact@patientfirstai.com.

My doctor can’t print out my vaccine records. How should I send you my information?

Print out the paper version of the vaccine form and have your doctor fill it out with the information they have on hand. Then submit that form via the Patient First.AI app.
