About the Varsity Student Athlete Physical Examination Form

The Varsity Student Athlete Physical Examination Form is only required by students participating in their first year of varsity sports. If you plan to participate in varsity sports, NCAA rules require a physical examination within six months of the first day of participation in the sport. Students are also required to include proof of a sickle cell blood screening test. Parents cannot perform the physical examination or sign the form, even if they are physicians or nurse practitioners.
