Student Medical Forms Information

All information must be received before the following deadlines to avoid a registration hold.

Term:

• Fall: September 1, 2024
• Spring: February 1, 2025
• Summer: July 1, 2025

All new students must:

Complete the New Student Immunization and Tuberculin form, regardless of age or insurance status. This includes undergraduate students, graduate students, and students who are returning to MIT after a leave of absence of a year or more. Your proof of vaccinations and/or required laboratory reports must be uploaded when you submit the form.

If your doctor requires a paper form to fill out, you can print out the 2024-2025 New Student Vaccination and Tuberculin form, and then submit that form via the Patient First.AI app.

Varsity student-athletes must: 

Complete the Varsity Student-Athlete Physical Examination form in addition to submitting the 2024-2025 New Student Vaccination and Tuberculin form.

Health Science and Technology (HST) students must:

Submit the following documents:

• proof of vaccinations including positive titer results for the following: measles, mumps, rubella, hepatitis B, and varicella
• tuberculosis screening test (either ppd or IGRA) done within six months of enrollment

• Students requiring an exemption for religious or medical reasons must submit the exemption form.
