What forms are required for me as a varsity student athlete?
All first-year intercollegiate varsity athletes are required to fill out the Varsity Student Athlete Physical Examination Form (available in late Spring 2026) in full and submit the form to health.mit.edu/athletics.
Can I get a Sickle Cell Trait Waiver Form?
No. The NCAA requires all varsity athletes be tested for sickle cell trait. The Sickle Cell Trait Status form is part of the Varsity Student Athlete Physical Examination form (available in late Spring 2026).
How do I submit the athletics forms?
Once you have completed the Varsity Student Athlete Physical Examination form (available in late Spring 2026), go to health.mit.edu/athletics and follow the instructions.
