How do I update my legal name and/or sex in the MIT Health electronic medical record?

MIT Health uses information from the MIT Registrar, MIT Human Resources (HR), and health insurers when we first create your electronic medical record (EMR). If you later change your legal name or sex with the Registrar, HR, or your health insurer, your EMR at MIT Health will not automatically update. Use this EMR update form to let us know about changes you have already made elsewhere, and we will make sure your EMR is updated.

If you are an employee at MIT Lincoln Laboratory, Draper, Whitehead Institute, or Broad Institute, you will need to make changes for yourself and covered family members with your individual HR offices. If you are a family member covered by the MIT Student Health Insurance Plan (SHIP), you will need to make changes to your legal name or sex through MIT SHIP. You need to do this before you use this EMR update form to request changes in the EMR.

How do I update my gender, pronouns, or preferred name in the MIT Health electronic medical record?

Use this EMR update form to let us know about any changes you would like to make to your gender, pronouns, or preferred name in the EMR. 

How do I update my sex and/or legal name with my health insurance?

It is important that the sex in your MIT Health EMR matches the information in your health insurance record. If it doesn’t match, your insurance might incorrectly deny an insurance claim.

  • Students covered by the MIT Student Insurance Plan (SHIP) and employees and family members covered by an MIT-sponsored health plan only need to make these changes with the MIT Registrar or MIT Human Resources. The changes you make there will be automatically sent to Blue Cross Blue Shield of Massachusetts (BCBSMA).
  • If you are an employee at MIT Lincoln Laboratory, Draper, Whitehead Institute, or Broad Institute, check with your individual HR offices to see if they automatically forward changes to BCBS; if not, you may need to contact BCBS yourself. If you are a family member covered by the MIT Student Health Insurance Plan (SHIP), you will need to make changes to your legal name or sex through MIT SHIP. If you are a student covered by another insurance plan, you will need to contact your health insurance company directly.
