What about my medical record?

Incoming students often ask about transferring their medical records to MIT Health. But for most new students, a completed Student Medical Report Form will provide all the information we need.

However, if you have a complicated medical history or a chronic medical condition for which continuity of care is a concern, your new primary care provider (PCP) will need access to a more complete medical record. In that case, you can you can either bring a paper copy of your medical record to your first appointment or meet with your provider to discuss the records that would be helpful, and sign a release to have them sent to us at that time.
